Frequently Asked Questions (FAQ)

We’ve compiled answers to some of the most common questions from officers to help you access important information quickly and easily. Here, you’ll find guidance on payroll, time off, and other employment-related topics. If you can’t find the answer you’re looking for, please reach out to your Shift or Site Supervisor.  We are always here to help!

Processing Deadline

All direct deposit requests (with the required documentation) must be received by 10:00 a.m. Eastern Time each Monday for the deposit to process on the upcoming Friday paycheck.

Requests received after 10:00 a.m. ET on Monday will take effect on the following Friday’s paycheck.

Required Documentation

Each request must include:

  • A fully completed Direct Deposit Form. (These can be found at your site location)
  • Supporting bank documentation (voided check or official bank letter).
  • Handwritten information is not accepted.
  • The document must clearly display the employee’s name, address, routing number, and account number.
  • Both the form and the supporting documentation must be submitted together in the same email.

Split Deposits

To request a split deposit, complete Bank 1 and Bank 2 sections on the form.

Select “Yes” for split deposit and provide a dollar amount for one account. The remaining balance will be deposited into the second account.

Important Note

We cannot accommodate percentage-based splits.

Submitting the Information

You will submit the Direct Deposit Form and the supporting bank documentation to the Account Manager at [email protected] titled with your name and Direct Deposit Change Request.

PTO Request

All PTO requests are subject to approval by site leadership and are reviewed on a first-come, first-serve basis. Requests should be submitted at least two weeks (14 days) in advance.

Approval is not guaranteed and will depend on staffing needs and operational requirements.

Unpaid Time Off Request

All PTO requests are subject to approval by site leadership and are reviewed on a first-come, first-serve basis. Requests should be submitted at least two weeks (14 days) in advance.

Unpaid time off requests must follow the same process and are also subject to approval by site leadership; however, PTO requests will take priority over unpaid time off requests.

Priority System

PTO requests are processed before unpaid time off requests to ensure fair allocation of paid time off benefits.

Submitting The Time Off Request

Employees are required to complete a PTO/Vacation Request Form and submit it directly to site leadership on site for review and approval at least two weeks (14 days) in advance.

Submission Process:

Step 1: Complete the PTO/Vacation Request Form

Step 2: Submit directly to site leadership on site

Step 3: Allow at least 14 days for review and approval

Step 4: Wait for confirmation from site leadership

📋 Required Timeline

All requests must be submitted at least 14 days in advance. Requests submitted with less notice may be denied due to operational requirements.

Required Documentation

Each request must include:

  • A fully completed and signed IRS Form W-4 (current year). (Download the form from the IRS website.)
  • (Optional but recommended) Withholding worksheet or estimator printout if you used the IRS Tax Withholding Estimator.
  • Handwritten forms are not accepted; please complete the fillable PDF and e-sign or print and sign clearly in ink, then scan.

💡 Tip

The W-4 lets you adjust filing status, dependents/credits, multiple-job settings, other income/deductions, and any extra amount to withhold each paycheck. Submit a new W-4 whenever these change.

How to Complete Your W-4

  1. Download the latest Form W-4 (Employee’s Withholding Certificate) from the IRS: Form W-4 (fw4.pdf)
  2. Fill out Steps 1–5 per the form instructions (use the IRS Tax Withholding Estimator if needed)
  3. Sign and date the form (digital or ink signature). Unsigned W-4s cannot be processed.

State Withholding (if applicable)

If your state requires a separate withholding form, include the state W-4 along with your federal W-4. (Check your state revenue site for the latest form.)

Submitting the Information

Email your completed documents to the Account Manager at [email protected] with the subject line:

Attach in the same email:

  • Completed and signed IRS Form W-4 (PDF)
  • Any applicable state withholding form (PDF)

Important Note

If you intended direct deposit changes instead, use the Direct Deposit instructions; for tax withholding changes, always use Form W-4.
